• A copy of athletes Birth Certificate
• An up-to-date photo of your athlete(s)
• Insurance card
• Updated Physical (Check with HealthPoint for Free or discounted physicals) *Ron Savignano will be at the field on July 19th at 5:45.
• The NKYFL Contract (this will be handed out the week of 7/17/2023)
• REGISTRATIONS & PAYMENT DUE ON THIS DATE (7/17) IF NOT ALREADY PAID IN FULL
• Paperwork and Fees are due no later than July 20, 2021.
