Football Fee: $200.00 per player (includes $130.00 fee + $20 equipment deposit and $50 volunteer deposit). Cheerleader Fee: $145.00 per cheerleader plus a cheer package (includes $75.00 fee + $20 equipment deposit and $50 volunteer deposit). Flag Football Fee: $75.00 per player (Includes $25.00 fee and $50 volunteer deposit). Spirit Squad Fee: $95.00 per player plus accessories (includes $25 + $20 equipment deposit and $50 volunteer deposit). *NOTE: A 3.5% processing fee will be added for all online payments. *NOTE: A 6.0% KY sales tax will also be collected for all sports registrations. * Make checks payable to “Spartan Youth Football”. * Only one $50 volunteer fee is required per family.
0 I understand that participation in the Spartan Youth Football organization requires our family to provide 6 (SIX) volunteer service hours as indicated in the Spartan Family Handbook to support the organization or I will forfeit my $50 volunteer deposit. 0 I understand the registration fees are non refundable after the first two weeks of practice. Requests for reimbursement must be made in writing to the Spartan Board of Directors. 0 I agree to take care of the equipment & uniforms (items). I agree to return the items in the condition received or I will forfeit my equipment deposit. Failure to return the items will also result in the player / cheerleader non-admission to the Spartan Banquet. 0 I hereby grant permission to Spartans Youth Football, and its coaches, Board members, team moms, and assigns (hereinafter “The Spartans”) to photograph my image, likeness, or depiction and/or that of my minor children (if applicable). I hereby grant permission to The Spartans to edit, crop, or retouch such photographs, and waive any right to inspect the final photographs. I hereby consent to and permit photographs of me and/or those of my minor children to be used by The Spartans worldwide for any purpose, including educational and advertisement purposes, and in any medium, including print and electronic. I understand that The Spartans may use such photographs with or without associating names thereto. I further waive any claim for compensation of any kind for The Spartans use or publication of photographs of me and/or those of my minor children (if applicable).
